1. I, [Employee Name], request an Interest Free Season Ticket Loan from the Diocese of Westminster in the sum of £ [Loan Amount £] ([Loan Amount in Words])
2. I confirm that the Diocese will be entitled to recover from me the amount of the Loan over 12-month instalments. The deductions are to be made from my monthly salary commencing [Start Date] until loan is fully repaid.
3. I further confirm having agreed that if, for any reason, my employment with you is terminated;
- I will immediately repay to you the amount (if any) of the loan outstanding by the date of termination, and,
- You will, in particular, be entitled to deduct such amount from any sums due from you to me at such date (i.e., accrued salary).
4. I confirm that I will provide the season ticket receipt/proof of purchase within 2 weeks of receiving the loan.
By failing this, my loan may be withdrawn.
